What to Expect

When you come for your first appointment, the therapist will ask you what brought you here and what symptoms you are experiencing. You can tell the therapist whatever you think they will need to know to be able to help. The therapist will listen and answer any questions you might have. By the end of the first session you will have a better understanding about how the therapeutic relationship will work and what type of treatment you will receive.

Together, you and your therapist will develop a treatment plan and jointly decide how often you will meet, what time will be most suitable, and what other information might be needed.

We welcome your calls to schedule an appointment or to request further information between the hours of 9:00 a.m. and 5:00 p.m. Our therapists schedule both daytime and evening appointments and they will work with you to arrange a time that will be best suited for you or your family.

When you call to schedule an appointment you will be asked to provide some basic information such as your name, phone number, address, birth date, and your health insurance information. Many insurances will pay for all or a portion of your mental health care. We will do our best to work with your insurance company in order to determine your benefits prior to your first visit and we will pass that information along to you. We also encourage you to contact them as well.

When you have arranged for an appointment you will be asked to complete several intake forms. You may download them from below (requires Adobe Acrobat) pick them up at the office in advance or request that we mail them to you. You may also complete them when you come for your first appointment, however we ask that you come in at least 25 minutes prior to your appointment time so that you will not be rushed. If you have any questions you are encouraged to call or ask the receptionist as you go along.

All patients must review and sign the following forms. If the patient is under 18 the parent must give consent for treatment and complete the following forms in the child’s behalf.
